  • Overview

Over 80% of older Australians have at least one chronic disease. Subsequent increased healthcare utilisation and burden of care, means heavy Physiotherapist involvement in recovery from hospitalisations including assessment, treatment and discharge planning. This presentation will cover concepts of chronic disease, multi-morbidity, comorbidity and frailty for diverse populations, including those who identify as ATSI, and evidence for their Physiotherapy care.
