  • Overview

Managing Complexity Across the Continuum: Metro, Regional and Community Care of a Patient with a Home-Ventilated Tracheostomy with Post-Polio Syndrome. This case presentation follows the complex journey of a patient with post-polio syndrome and a long-term tracheostomy. It explores the physiotherapy management from acute to subacute in tertiary, regional, and community settings and will discuss the challenges of ventilator weaning, discharge planning, and transition to home. The presentation demonstrates the importance of collaborative care, clinical reasoning, and continuity of physiotherapy management in supporting safe, patient-centred outcomes across the continuum of care.
